Official Documentation of a Confederate Soldier's Death

I have a situation where a soldier's official record says he died of disease at Richmond Hospital and his widow's Confederate Pension Application states he died at Camp Point Lookout. Does this type of discrepancy arise often? Which is usually correct? Thanks for your help.
