If you're referring to the Social Security Death Index, the Washington, DC means that it was the last mailing address that they had for him, which may not be the place that he died. For example, I have a relative who lived in Washington, DC all of his life. He died, however, in Bethesda, MD, because that's where Bethesda Naval Hospital is.
