Donnell name change?

My Gr Grandfather Daniel Donnell born in Charlestown, Ma. also had the name O'Donnell on his birth record and all future records I have found only show the Donnell name. I am curious as to how a person went about a name change in the late 1800's and was there some legal record of name changes and where was it kept?
