First there are no middle names in Germany. You have one or more first names and one of them is used in everyday life. Indeed it seems there is no common rule for whole Germany and all times. So I can give you only my personal experience. In earlier times it was often use to give the names John (boys) and Maria (girls) plus a second fist name. In this case the second first name was used as "calling name" in everyday life.
