I picked this message up by incident, but I can tell you on your name "Adama i Anny Lewickiej", that the correct form would be Adam Lewicki and Anna Lewicka. "Anny Lewickiej" is a grammatical form, it means "of Anna Lewicka". Also Michal, syn Adama, would be "Michal, son of Adam". Lewicki is male grammatical form, Lewicka is female, but itīs the same family.
