St. Charles (Illinois) Chronicle, 15 July 1904:
Obituary
Mrs. Henry Hazelhurst died in Fall City, Washington, on July 4, of paralysis, after an illness of two months. The body was brought to St. Charles and the funeral was held Sunday at 2:30 at the home of Richard Hazelhurst, Rev. F. M. Campbell of the Free Methodist Church officiating. Interment was in the North cemetery.
Annette Minard Hazelhurst was born July 22, 1846, on a farm north of St. Charles. Her father, Geo. W. Minard, was an old settler, a brother of Ira Minard, one of the founders of St. Charles. She was married to Henry B. Hazelhurst February 8, 1865, and they resided in this city on a place just east of the Great Western depot. It was sold and the last years they spent together was in a home on the electric road toward the North cemetery, where Henry died nearly four years ago.
Two years ago, accompanied by her son Fred and family, she moved to Fall City, near Seattle, Wash., where she passed from the scenes of earth July 4, 1904. Her father and mother were Methodists and she held to their belief, having been converted about six years ago, and lived a prayerful life, having a hope in the hereafter.
She had one half sister who resides in Elgin.
Of her three children, two little girls died in infancy, and her son, Fred, who accompanied the remains here for burial, is left with his faithful wife and two children.
