Brenda, You can request copies of SS Applications on the Social Security web-site. Once you have the site up go to the Search field and type "SSA-711". On the next screen select "Social Security on-line Guide to FOIA Request". From there you will be able to find the request form. If your request does not have a SS number it will cost you a little more. Applications made in 1910 or before may not contain the names of parents or place of birth. My ancestor was born in 1873 and applied for his number in 1937 so it did contain the parents and place of birth information. Good luck, Bev Notify Administrator about this message?
