Searching for the death record of Hannah W. Closs of Phelps, NY in 1912

Searching for the death record of Hannah W. (Hanes) Closs who died on September 23rd or 24th, 1912 in Phelps, Ontario Co., New York. She was 81 years old at the time of death (she was born July 1831) and was buried in the South Lyons Cemetery in Lyons, Wayne Co., NY. She was married to William H. Closs who died in 1914. Any info on her death record would be much appreciated. Thanks in advance for your help!
