Death certificates are maintained by the municipality (cities, towns for hamlets and villages that do not want to do this, and villages) where the death occurred and this might be different form where the person had been residing immediately prior to passing.

Assuming that the death actually occurred in the Hamlet of Wyandanch and not in some other community, then you would want to contact the Town Clerk of the Town of Babylon:
