Hi!
If you google ohio death index, you will be taken to the Ohio Historical page, where you can search the index. It does not show you the actual certificate, but gives you an option for ordering. If what I read was correct, the fee would be $5.25, and this is for a copy only, not a certified copy.
The only Andrew/Andy HAWKINS I saw that was killed in Cuyahoga county, was a date of January 15, 1936. That may be why the library never responded, IF it IS the same guy you're looking for, then the date was wrong.
