I neglected to mention that the OH Death Certificate information stating that John was from Ohio was for his son named Phillip M. Offutt. The daughter of John E. Offutt, named Deborah also died in Ohio, her death certificate stated that John was born in Martha's Vineyard, that information may be incorrect.
