Doug Corbin provided a break through making it possible to find the birth record for Ruby Hazel Johnston.
Below is a link to the Laurel County Kentucky Genweb. Do a search for the name “Johnston” and you will find this birth entry:
Ruby H. Johnston, b. 28 January 1917. Mother’s name: Bessie Poore. (I believe that was probably a misreading of the name “Tressie Poore.) At the rootsweb Family Search site is this entry:
Theresa Poore, father’s name David Huffman Poore. There is information that when he died in 1940 in TN, he had a daughter, Mrs. Theresa Johnson, living in Oklahoma. It would be easy to mistake Johnston for Johnson.

Unfortunately, it does not list the father’s name in this entry, but you can send for Ruby’s birth certificate. I believe that Doug Corbin has found the correct family and he has provided her father’s name.
I did not find the birth entry for Ruby’s brother or for her sister, Mary.
