Hi Dave,
I was only able to locate the 1920 and 1930 U.S. census records for Andrew (or Anthony) Shizak/Sizak.I could not locate his 1910 U.S. census or WW I Draft Registration Card.Where did you get the September 6, 1881 date of birth for Antoni?
Andrew’s surname could have been Syczak or even Szyczczak before it was changed.

I assume it is the Russian composer Peter Tchaikovsky that has the same sounding surname as your grandfather’s mother’s maiden name.Again you can use the above site to try various spellings such as “C*jkowski” for Czajkowski and Chajkowski,
Czajkowski is a common Polish surname with 22,131 adults having that surname.
