Joan,

Since she last appears in 1910 in Rhode Island, there is no gravestone transcription for her in the RI Cemetery Transcription database, so she may not have died in Rhode Island at all. That leaves you with an 8 year window for your search. If Flavien appears as a widower in Fall River, MA then it is possible she died in that city sometime between 1910-1918. You could write to the Fall River City Hall or Massachusetts Vital Statistics to find her death record, or contact AFGS in Woonsocket, RI to search their records. Just a couple of suggestions for you.
