That is my grandmother's death certificate. The parents shown on the certificate are her aunt and uncle who raised her. She was born in 1889 and by 1900 she is with her aunt and uncle so it's hard to know who her parents were.
Here is a link to a small tree I put up. I'm really trying to find out who the J. J. or Jacon Smith is. I found his son's death certificate and his father's name is stated as Jacon Smith.
